Eligibility Criteria

To be eligible to attend Vision25, applicants must meet the following criteria:

  • The young adult must be under the age of 25.

  • The young adult must have an Education, Health and Care Plan (EHCP), or a Social Care Plan from the Local Authority.

  • There should be an allocated social worker or relevant professional who can liaise with us regarding funding arrangements, unless the placement is privately funded.

Safety and Duty of Care

Vision25 has a duty of care to all young adults attending the centre, as well as to staff members and visitors. For this reason, we are unable to accept referrals for individuals who display challenging behaviours that may pose a risk of physical harm to themselves or others.

This policy ensures that we can maintain a safe, supportive, and positive environment for everyone accessing the service.

Please note that as of March 2026, our waiting list is currently around four years due to the high level of interest in our services. We appreciate your patience and understanding.

When a place becomes available, we will contact you to begin the next stage of the process, outlined below:

Vision25 Transition and Application Process

  1. Preliminary Application - The preliminary application form is emailed, completed and returned

  2. Day Service Visit - You will then be asked to visit the day service with your son/daughter. 

  3. Main Application - The main application form is completed.  This is more detailed.

  4.  ‘Getting to Know You’ – The young adult is invited to attend the service on a Saturday, accompanied by a parent or guardian. During this stage, the parent or guardian retains full responsibility for the young adult. Vision25 does not provide staffing support at this stage, and no fees are incurred. The duration of this stage is determined on an individual basis, according to the specific needs and readiness of the young adult.

  5. Initial Transition Period - The young adult continues to attend the service on a Saturday. Vision25 will provide staffing support at this point. Appropriate funding must be in place at this point.     

  6. Transition to Weekday Attendance – The young adult will continue attending on Saturdays while their attendance is gradually extended to include weekdays over an agreed period. The number of days will increase incrementally as part of this transition. Funding must remain in place for all staffed sessions.

  7. Transition Completion – Following successful completion of the transition period, a 3–6 month probationary period will commence. This period may be extended where appropriate to ensure the placement continues to meet the young adult’s needs.

Vision25 will discuss potential timelines with parents or carers and will remain in contact throughout the process, providing updates as appropriate. These timelines will include all of the stages outlined above.

During this period, we will complete the necessary preparations, including emergency care plans, medication plans, feeding plans, and any required risk assessments. All documentation will be reviewed and signed off by the parent or carer. We will also liaise with schools where necessary.
