We are an independent provider of specialist bespoke programmes for young adults (18 - 25) living with a learning disability. We work with young people with varied abilities and levels of needs, from mild to severe and profound and multiple learning disabilities.

 

Our service is located in Stockton-on-Tees.

We believe that individuals with learning difficulties are unique in their needs and abilities and our innovative approach specialises in providing a structured therapeutic environment that centres on personalised bespoke pathways to a more independent style of living.

Our Aims & Positive Outcomes

OUR AIMS are:-

 

To provide positive outcomes for people with a Learning Disabilitiy through person centred programme, experientail interventions and robust training pathways.

We achieve these by:

 

  • Ensuring that the client’s needs and wishes are at the centre of their bespoke programme.  

  • Providing a safe, secure and nurturing environment to help clients to develop independent living and social skills

  • Clinical and therapeutic teams work holistically to provide robust support and intervention

  • Ensuring our staff receive the highest level of training and career support to give them the right skills to meet clients’ individual needs

  • Supporting clients to attend work experience ,

  • Enabling clients to develop the skills to gain and keep employment

  • Promoting a healthy physical and mental environment that contribute to clients’ independence, fulfilment and overall well-being

Positive Outcomes

 

We work closely with Learning Disability experienced staff to provide innovative bespoke pathways, which enable individuals to achieve positive outcomes including:

 

  • Improved emotional and mental well-being

  • Increased personal choice and control

  • Improved social skills

  • Better independent living skills

  • Reduction in risk of harm to self or others

  • Enhanced personal dignity

  • Well maintained health and physical well-being

  • To enable each client to move on to least restrictive setting, as soon as appropriate.

We are unique in our ability to provide tailored bespoke programmes for each of our members that enables choice and control in their life as possible.
 
Members are supported by our experienced staff and multi-disciplinary team, which include Occupational Therapists and Speech and Language Therapists.  This enables individuals to be supported through the pathway as their abilities increase and needs lessen.

 

By encouraging individuals and giving them the support to take control in their lives Vision25 creates pathways that support individuals in their transition to more independent living. They deliver a person-centred and positive independent model of living to help their service users get the best possible quality of life possible.
